GPs may charge a fee for work that falls outside NHS services. This includes private medical insurance forms, private sick notes, holiday cancellation reports and other non-NHS letters, forms and reports and medicals. Fees start at £50 and vary according to the time involved, the complexity of the request and the professional responsibility undertaken. Our current fees are set out below.

PLEASE NOTE –

Payment – Payment must be made in advance by cash or cheque.

Fees – charges for complex reports may vary.  We will confirm any additional cost before starting the work.

Services not listed – Please ask at reception.  We will confirm whether the practice can provide the service and, where appropriate, give you a quotation.

Passport and driving licence applications – We do not provide countersignatures for these applications.

Private services fall outside our NHS contractual obligations. The practice therefore reserves the right to decline any request.

Timescale – Please allow up to 28 calendar days for private forms, letters and reports. The timescale begins once we have received all required information, appropriate consent and payment.  NHS clinical care must take priority, and complex reports may take longer.  We will let you know if additional time is required.

WE DO NOT PROVIDE

Passport and driving licence applications – We do not provide countersignatures for these applications.

Fitness for exercise or gym activities – Your fitness to participate should be discussed with the gym or activity provider.

Fitness for travel or flying – We do not provide certificates guaranteeing that a patient is fit to fly. Please contact your airline to confirm its requirements. You may need to approach your treating specialist or a private aviation medical service.

Housing letters – Councils have their own processes for obtaining relevant medical information and do not usually require a letter supplied by the patient, including where medication is involved.

Court appearances or probation letters – We do not provide these unless directed to do so by a court order. Your solicitor can advise you further.

Sick notes or letters for educational institutions – Parents or students are responsible for providing the institution with the required information. GPs are not obliged to provide these letters.

Benefit appeal letters – The Department for Work and Pensions has an established process for requesting relevant information directly from GPs.

Approval for dieting – A letter of permission is not required. We recommend a balanced diet alongside regular physical activity, where appropriate.

Fitness for specialist or high-risk activities – We do not assess fitness for activities such as parachuting, mountaineering, ballooning, triathlons, running, cycling or diving, as these assessments fall outside our expertise. Please contact the activity provider or an appropriately qualified medical service.

Fitness for procedures, operations or dental treatment – The clinician or organisation carrying out the treatment is responsible for assessing whether you are fit to proceed.

Witnessing signatures or confirming identity – These services can be provided by other suitably authorised non-medical professionals.

Testamentary capacity, powers of attorney and Court of Protection matters – These are specialist medico-legal matters with significant implications for patients and their families. We recommend that your solicitor obtains an opinion from an appropriately qualified expert.
