All requests for same day appointments will be taken from 8 am onwards. There will be no separate allocation of appointments at 2 pm. Please do not wait until 2 pm to request a same day afternoon appointment.
You will be asked the reason for your appointment. This will help us to prioritise your call and enable us direct you to the appropriate person.
You will be offered a telephone appointment in the first instance. The doctor will then decide if you need to be seen in person and will arrange the appointment for you.
If you feel your problem is urgent please let the receptionist know.
You are still be able to pre-book appointments. Please book ahead if you can.
We offer pre-bookable evening telephone consultations between 6.30 and 7.20 pm daily and pre-bookable nurse appointments between 7.30 and 8 am on one day per week (usually Friday). Evening and weekend GP and nurse appointments are available to attend at Amber Valley Health Hub at Church Farm Medical Centre in Ripley.

Preference of Practitioner
Whilst registered with us we encourage patients to see any of the doctors and clinical staff. However, they may also if they wish express a preference to receive services from a particular doctor. Once a preference has been indicated we will endeavour to comply with the request. However, there may be times when this is not possible, for example; doctor unavailability, or the services requested are not provided by the preferred doctor. Please also note that should a patient wish to see their preferred GP then it may result in a longer waiting time for their appointment.
Returning Telephone Calls
If you ask one of the doctors or staff to telephone you back and your telephone has been programmed not to accept hidden numbers then please disable this facility temporarily otherwise we will not be able to return your call.
If possible please try to telephone reception before 10:00 if you require a home visit.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed.
You can be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
Cancellations & Reminders
Cancel an Appointment
It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters may be removed from the list.
You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance. (excluding weekends and public holidays). You can also text to cancel your appointment.
Late For Your Appointment
Please attend your appointment on time, if you are late you may not be seen. If you are not seen you will not be able to rearrange your appointment until the next working day-except in the event of an medical emergency that requires immediate attention.
If you require an interpreter to attend with you when you see your Doctor please notify the receptionist and she will arrange this for you.
Text Reminder Service
We have a texting service which allows you to receive confirmation and reminders about your appointments.
To have this service you will need to register by completing a consent form.
Please remember to update your contact details with us when you change address, telephone numbers and email address.
Extended Access AVH
We are pleased to announce that The Park Surgery, Heanor is now part of Amber Valley Health Limited (AVH) along with the following practices: Crich Medical Practice, Ivy Grove Surgery, Jessop Medical Practice, Kelvingrove Medical Centre, Parkside Surgery, Somercotes Medical Centre, and in collaboration with Ripley Medical Centre and Brooklyn Medical Practice.
We aim to improve the service and access to GPs and will be offering extended appointments, late evening and Saturdays.
These will be at the Amber Valley Health GP Hub at Church Farm Primary Care Centre in Ripley. DE5 3TH.
Please note, the vast majority of AVH GPs and staff members are from one of the local practices named above.
The Amber Valley Health GP Hub is currently open for pre-booked appointments only.

We are fortunate to be recognised as a training practice and we usually have foundation year doctors and specialist training registrars attached to the practice. They stay with us for between four months and a year to gain further experience of general practice and complete their training to become general practitioners. On some occasions when you see a doctor in training, you may be asked before your appointment whether he/she can video their consultation with you. This is to help assess and improve their consultation skills. You are under no obligation to say yes. If you feel you are happy to be videoed, further information and a consent form will be given to you.
At times we also have medical students from both Nottingham and Derby Medical Schools attached to the practice to gain firsthand experience of general practice. It is very valuable experience for them to sit in during your consultation with the doctor. However, if you would prefer them not to be present then please let the receptionist know when you arrive.
Because we are a training practice we have to be inspected regularly to ensure we continue to meet the high standards necessary to train competent doctors. At such a visit a small number of patient records may be inspected for the purpose of ongoing accreditation and quality assurance activities.
